The My Home Hemo app is a free service for dialysis patients in Australia developed by Vulsen. This page is used to inform website visitors regarding our policies with the collection, use, and disclosure of Personal Information if anyone decided to use our Service.
This app is part of a clinical improvement initiative of the Western Renal Service, supported by the University of Sydney and
Nepean Blue Mountains Local Health District. Using this application is entirely voluntary and will replace the paper-based dialysis
information in a diary or notebook that you would otherwise collect.
Should you, as the user of the application have any dialysis related issues that you may wish to discuss with
your healthcare team, you should continue to contact them directly.
This app allows you as the user to electronically enter and store data related to each hemodialysis session that you conduct at home.
It also allows you to view, monitor and print trends in various hemodialysis-related
parameters including body weight, blood pressures, arterial and venous pressures and blood flow rates.
This app does not collect any additional information outside of the usual parameters entered for each dialysis session.
The data that you enter into the application belongs to you and you may choose to share the data by printing on a paper based format
or electronically in real-time with your healthcare team. Sharing this data either electronically or printing the trends in various hemodialysis
related parameters may assist your healthcare team to resolve some of your dialysis related issues. Sharing the data electronically in real time
with your healthcare team could allow preliminary assessment of your dialysis parameters without a face to face consult or home/clinic visit,
which may otherwise be required if this data is only available on paper in a diary or notebook.
Should you decide to share your data electronically in real-time with your healthcare team, the data will be uploaded and stored in a secure cloud
based server and will be available to your health care team to access at any time. However, your healthcare team will not be responsible to act and make
clinical decisions based on the data they may have access to, unless you request them to do so, each time.
The data stored within the cloud based server is accessible only to you and your healthcare team. No data is shared with any other third party entities or organization.
